About Our Client

This NHS Trust is expanded their Community Response Team in the Chesterfield and Belper regions. Offering support to those people who have recently left hospital and require short term/interim support in their own homes.

Job Description

* Supporting people in their own homes who have recently been discharged from hospital

* Short term/interim care packages

* Delivering patient care which may involve personal care

* Work as the wider Community Response team

* Provide delivery of treatment in line with each persons Care Plan

* Promote and encourage independence

* Ability to report any changes in condition, accidents or incidents

* Respect dignity and privacy

The Successful Applicant

* Previous experience in a healthcare setting is essential

* DRIVING LICENCE and Access to Own Car is essential for this position as you will be travelling around either Chesterfield or Belper (location closest to you)

* Excellent communication skills

* Flexible with shifts - The response team work 7 Days per Week

* Shifts - Earlies 0800 - 1400 and Lates 1600 - 2200

* Full Time 36 Hours per Week or Part Time minimum of 18 hours per Week (on a rota covering 7 days 0800 - 2200)
