Job description
To provide support, advice, information and education to ward staff and multidisciplinary teams to facilitate a safe and timely discharge back to the community. To undertake all aspect of clinical duties as an autonomous practitioner. The post holder will support nursing staff in obtaining a documented discharge care plan, discharge destination and estimated date of discharge for all inpatients. The post holder will be able to demonstrate knowledge of the legal framework under and duties relating to hospital discharge and other legislation relating to discharge planning and the reimbursement process.
