Job description


We are looking for a committed and dynamic experienced nurse including those from a deputy management background, to provide leadership and clinical direction on Trefynwy Ward in Monnow Vale Health and Care Facility.

The successful candidate will have a passion for person centred care and will work alongside the integrated service team and frailty service.

The successful candidate will have a proven track record of sound clinical leadership and will have a value base that is person centred and in line with current evidenced based practice.

They will be able to work alongside and support an inpatient multidisciplinary team to provide safe, effective, person centred care aimed at maintaining independence and person-hood. They will have excellent communication skills and have a passion for innovation and problem solving.

Monnow vale are looking for someone who takes pride in their role, has a passion for improving systems and ways of working in a collaborative approach. We are looking for someone dynamic and enthusiastic about change management.

The ward is a 21 bedded unit managed medically by the GPs, the ward runs as an integrated service facilitating direct admission from the community and working closely with the district nurse team in Monmouth. The patients range in age and vary in need from rehabilitation to complex discharge planning.

The ward has a friendly team who have a passion for providing excellent care for their service users.

The ability to speak Welsh is desirable but not essential for this role; Welsh and/or English speakers are equally welcome to apply.
