Job description
Observe and report the effects of all interventions to the responsible CMHN/Care Coordinator, through individual discussions, team meetings and clinical reviews. Use a range of methods to promote effective communication bothverbally and written methods including electronic systems, telephone calls, case recordings, etc. Maintain accurate and timely records of all contact with the individuals, carers and other clinical contacts. Work closely with a range of agencies, including Primary Care, Local Authorities, voluntary organisations and Housing providers. Ensure all incidents which may compromise Health and Safety arereported in an appropriate and timely manner. All employees of the Health Board are required to maintain theconfidentiality of members of the public and members of staff in accordance with HealthBoard policies. Overcome barriers to communication by adhering to person centered care planning. The post holder will be required to work within your level of competence and discuss any concerns with your line manager. At no time should you work outside your level of competence. Provide written and verbal reports based on facts and observations. Carry out base-line observations e.g. vital signs: temperature, pulse, blood pressure, respiratory rate, etc ensuring records are kept and results are communicated promptly to the responsible CMHNs or Care Co-ordinator. Accompany and transport patients to clinic appointments, if necessary/appropriate. Supervise the taking of medication, recognising side-effects andreporting promptly to the responsible CMHN or Care Co-ordinator. With guidance from the Community Team Leader and registered CMHN, be responsible for prioritising and organising day to day workload and activities. Be required to attend relevant meetings, training and supervision asindicated by the Community Team Leader. Organisation and facilitation of individual and/or group based activities with support of the Multidisciplinary Team. Driving and navigation for community work. Keyboard and information technology skills to complete accurate and timely case recordings, communication via email and intranet and e-learning. Will demonstrate a positive attitude towards patients well being which is based on commitment, hope and optimism. Develop rapport based upon a working relationship which maintains appropriate boundaries at all times. Work with the individuals and carers to meet the needs and achieve the goals identified in their individual care and treatment plans. Be aware of documented relapse indicators as identified in the careplan and monitor for early signs of relapse, reporting progress, level of functioning and mental state to the responsible CMHN/Care Co-ordinator. Provide regular and practical support to the individuals and their carers to enable them to develop and manage their own independence. Provide information on health promotion and healthy lifestyleprogrammes. Support and signpost individuals and carers to access a wide range of community resources based upon individual need. Participate in meaningful educational and leisure activities with individuals as identified in individual care and treatment plans.
