AKTON recruitment is working closely with the local authorities and we are looking for a Reablement Physiotherapist in the Southwark area. 3 - 6 months contract.

Job Summary

To perform specialised Physiotherapy (PT) assessments and interventions as a key member of the multidisciplinary rehabilitation and admission avoidance pathways. This includes managing clients' physical, cognitive, psychological and social abilities within a physical, social and cultural environment.

To be responsible for providing high quality evidence based rehabilitation to clients with a wide range of complex disabilities and needs, who may have multiple pathologies and co-morbidities.

To be responsible for holding a caseload of community clients within the Intermediate Care service using advanced clinical reasoning and goal setting to support the assessment and interventions provided using a client centered approach.

To be a source of Physiotherapy clinical expertise and advise therapy colleagues and other health professionals within the multi-disciplinary team and professionals working in external organisations.

To provide supervision and facilitate professional development of Band 5 Physiotherapists, Rehabilitation Support Workers, new or junior staff and Physiotherapy students as required.


Duties and Responsibilities

To carry out timely assessment, treatment and rehabilitation of community clients to prevent hospital admission or re-admission and facilitate early discharge home from hospital.

To be professionally and legally responsible for own practice and accountable for all aspects of physiotherapy treatment delivered to the client to ensure a high standard of clinical care.

To work as an autonomous lone practitioner, case managing clients in the community and coordinating their care.

To be able to identify significant health changes that would warrant medical intervention and liaise with the relevant practitioner.

To respect an individual's ethnicity, cultural and religious beliefs and adapt your physiotherapy assessment and treatment accordingly.
