Job description

Duties to include, but not be limited to: Provide a clinical pharmacy service to patients, wards and departments of the local Trust hospital(s) in accordance with clinical guidelines and professional, departmental and Trust policies. The clinical pharmacy service is based around medicines reconciliation on admission, clinical review and discharge processing all delivered as a team approach. Develop and apply your clinical knowledge to accurately and appropriately record individual clinical pharmacy involvement in safe and effective patient care. Provide professional and legal clinical supervision in the dispensary as allocated. To support the governance arrangements for safe medicines management including safe storage, incident reporting and implementing risk minimisation schemes. Act as a role model and to train and supervise other pharmacy staff as allocated. This may include undergraduate pharmacy students, pre-registration pharmacists, technicians, ATOs, work experience students, newly qualified pharmacists and other members of the multidisciplinary team.
