To provide Housing Support to Young People (16-25)
JOB DESCRIPTIONJob Title: Housing Support Worker (level 3)Project and Location: Housing Support for Young People in the Armagh/Dungannon/Magherafelt AreaReports to Project ManagerMain Purpose of Job: To provide Housing Support to Young People (16-25)Main Job Tasks
Provide Housing related Support to a case load of young people living in the community
Assess needs and develop support plans with each young person to meet their specific needs, ensuring support plans are continually monitored and reviewed to assess progress toward agreed goals
Develop, monitor and review risk assessments and risk management plans for each young person.
Maintain a Person-Centred approach at all times ensuring that young people have the necessary information they need to make informed decisions.
Develop professional working relationships with young people and provide support with all aspects of independent living including cleaning, cooking, budgeting etc
Support young people to apply for benefits and grants, to report repairs, to register and attend medical appointments, arrange transport and signpost to other organisations as appropriate.
Liaise with other agencies to ensure the young persons’ needs are met.
Maintain accurate, up to date records and case files in relation to all work activity and meet relevant deadlines as requested by the Project Manager.
Adhere to all policies and procedures as set down by BCM and comply with the standards for NISCC (Maintain NISCC Registration annually)
To promote Service User involvement and active participation in the Project, including liaising with service users on a 1:1 basis for consultations
To carry out marketing presentations when required
Attend local networking groups to promote BCM services.
To be available to work at times appropriate to the changing needs of the Service Users and to ensure the efficient use of staff resources. This may include (on occasion) working outside normal hours.
To carry out regular tests and reviews of all health and safety equipment and procedures relating to the Housing support service
Develop, monitor and review risk assessments and risk management plans for each young Person in a timely manner
PERSON SPECIFICATION – HOUSING SUPPORT WORKER
ESSENTIAL
Minimum of 2 years’ experience supporting Young People
Minimum of QCF level 3 in Health and social care
Excellent verbal and written communication skills
Ability to competently use IT for communication and record-keeping.
Ability to promote the service by preparing and delivering presentations to key agencies and partners.
Excellent time management / organisational skills
Possess a full (clean) UK driving license and business insurance and have access to a car (you will be expected to transport Service Users, on occasion to appointments etc.)
GENERAL TERMS AND CONDITIONSDuration: PermanentHours: 35 hours per week, (and may on occasion include evenings and weekends)Flexible working: Ability to start work between 8am-10am and finish between 4pm-6pm.Salary: £22,452.08 based on BCM Salary points 22-25 (£22,452.08 - £25,390.39)Holidays: Based on 21 days per annum plus 12 Public Holidays.
NISCC: You will be required to attain (if not already attained) registration with the Northern Ireland Social Care Council.
