Duties:
- The post will be to provide a robust risk assessment, care plan and safety plan within the Emergency Department within our acute Hospitals, for Children and Young people who have attended following an incident of self harm, or who are presenting with suicidal ideation/in crisis.
- The practitioner will assess and feedback to the community teams to ensure that each young person is receiving the ongoing treatment required.
To ensure that all CYP are supported or signposted on discharge and receive a 7 day follow up appointment.
- To ensure that all documentation following the assessment is clear and accurate.
To ensure that any young people who are deemed to need inpatient care are identified and the process is followed.
- To work closely with the Rapid Assessment and Intensive Support Team to ensure that all CYP are provided with support.
