Job description
CLINICAL: To deliver Alder Active programme to appropriate inpatients. To work closely with the Health and Wellbeing coach to provide supportto patients and families enrolled intoAlderActiveprogramme. Toassess and developappropriateexercise prescriptionprogrammeaimedforchildren and young people. Tosupply,distributeandreplenishstock of activity equipment. To liaise with theMDTand inpatient teams regarding patient care, highlighting any variations to expected outcomes. To be able to work without direct supervision in a variety of settings including,hospitaland in thecommunity. Use effective communication skills, verbal and non-verbal to communicateactivityprogrammesto patientswho may have barriers tounderstanding. To communicate sensitive information effectively with patients,caregiversand multi-agency staff andreport any relevant details to themulti-disciplinaryteamalways maintaining confidentiality. To be able to respond to questions from parents,carergiversand other agencies appropriately butrecognisethe need to report back to theMDTas necessary. To contribute to development of patient care by attending relevant team, departmental and other appropriate meetings. To effectively complete outcome measures and collect audit data to demonstrate service effectiveness To identify and evaluate areas for potential service improvement. GENERAL: To maintain own continuing professional development (CPD) and participate in the staff appraisal scheme and personal development review (PDR). To participate in in-service training programmes available via staff meetings, tutorials, training sessions, external courses and reflective practice by regular attendance and participation in the former including the delivery of presentations. Participate in clinical governance/ evidence-based projects/audits/research. To collect research data as required. To communicate effectively and work collaboratively with Allied Health Professionals (AHPs), social services, education colleagues and voluntary organisations. To demonstrate a sound understanding of Trust values, Clinical Governance and Risk Management and apply to the work situation. To attend Mandatory Training and incorporate these within working practices. To attend Child Protection training and work in accordance within the national guidelines. Adhere to the Chartered Society of Physiotherapy Standards for Physiotherapy Practice To maintain strict confidentiality at all times ORGANISATIONAL: To be responsible for own workload and to plan and organise efficiently and effectively with regard to patient management and time management. To be aware of Health and Safety aspects of your work and implement any policies, which may be required to improve the safety of your work area and ensuring that equipment use is safe. To be responsible for equipment used in carrying out physiotherapy duties, and to adhere to departmental policy, including competence to use equipment and to ensure its safe use by others through, teaching, training and supervision of practice. To maintain accurate and up-to-date documentation, in line with legal and departmental requirements. To have knowledge of and comply with the organisational and departmental policies and procedures.
