Job description
The post holder will support the CHC Team by providing a coordinated approach to the operational functions of the Front Door. This will include the management of new referrals received via the CHC portal, liaising with the wider communities and other professionals as required, and progressing referrals through the CHC assessment process. The role will also include any duties commensurate with a Band 5 clinical role, such as assisting with Fast-Track referrals, case management, FNC reviews and applying the National Framework for Continuing Healthcare across Sussex. This will be in line with the pan-Sussex standard operating processes and procedures and ensuring compliance with the National Framework for NHS Continuing Healthcare and Funded-Nursing Care.
