Job description

* To work as an integrated member of a Therapy Team delivering person centred care with particular focus on clinical and rehabilitation interventions, enabling individuals to achieve their optimum physical, psychological, social and emotional wellbeing * To independently assess patients referred into the service / speciality to determine clinical diagnosis and treatment indicated and maintain records as an autonomous registered therapist within a variety of clinical settings * To manage a clinical caseload to a high professional standard and seek guidance from senior colleagues when necessary * To provide patients and their carers with a therapy plan that promotes the development needed to acquire the physical skills to live as independent a life as possible and reach their maximum potential * To work as an autonomous registered therapist and be responsible for the Occupational Therapy management of patients referred into the service. * Ensure that patients are seen within agreed waiting times. * To work towards and achieve role specific competencies. * To work as part of the multi-disciplinary and multi-agency team and in partnership with other services and agencies including Social Services and to ensure that occupational therapy is fully integrated into the patients care. To coordinate team multi-disciplinary meetings. * To communicate effectively with other health professional and outside agencies where appropriate. * To participate in clinical supervision and training. * To contribute to the service targets and key performance indicators to reduce length of stay, monitor and assist with patient flow across therapy pathways, preventing unnecessary hospital admissions and effective discharge process by providing timely community based rehabilitation and community care. * To maintain high quality standards in all aspects of care delivery, working in partnership with primary care colleagues, other health professionals, patients, carers, statutory, independent and voluntary organisations. * To provide training and leadership to student physiotherapists, junior therapists, therapy support staff, peers and other health and social care colleagues. * Participate in service development activities including audit and research
