Danesbury is an Inpatient Neurological Rehabilitation Unit providing rehabilitation to thoses who have suffered an accute stroke, brain injury or other neurological conditions. We are looking for a Progress Chaser to join our friendly team of nurses, doctors, physios, occupational therapists, clinical psychologists. Within this role you will support the multi-disciplinary team to progress the patient’s pathway thereby reducing any delays for the patient and working toward the patients Expected Discharge Date. You will have daily interaction with the team and the unit's allocated social workers.

In addition to the basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a minimum payment of £1,192 per annum (pro rata where applicable).

To support Inpatient Units to progress the patient’s pathway thereby reducing any delays for the patient and working toward the patients Expected Discharge Date.

• To follow up and progress actions to deliver the timely discharge of patients to meet their expected date of discharge.

• To ensure the delivery of high standards of patient care at all times, in line with Trust policies and guidelines.

• To work as an effective member of the multi-disciplinary team.

• To work within the boundaries of the role and refer all other matters outside of their scope of practice to the Registered Nurse or Therapist.

• To observe high levels of confidentiality at all times.

• To manage and prioritise own workload within deadlines.

• To record all types of information accurately and have the ability to produce and maintain documents and spread sheets.

• To work as part of the multi-disciplinary team, ensuring effective communication across all departments.
• To communicate effectively with patients, carers, visitors, other staff members and other agencies at all times to ensure all are aware of pathway progression and discharge plans.
• To attend daily patient meetings, ward rounds and MDT meetings to record and take actions to expedite investigations and treatments to support pathway progression.
• To progress patient discharge as early in the day as possible.
• To work within the Red to Green model and to escalate constraints that cannot be resolved.
• To update the patient information board to ensure accurate timely information is communicated to the multidisciplinary team.
• To develop communication channels with the multidisciplinary team, including Allied Health Professionals, Medical Staff, Nursing Staff and Social Workers and Support Teams to ensure actions identified at the daily patient meetings are progressing. This can be of a sensitive and confidential nature.
• To escalate to senior staff any barriers that may result in a delayed transfer of care.
• Use SBAR (situation, background, assessment and recommendation) to review actions and constraints daily and identify how any blocks to progress can be addressed.
• To liaise with the nursing, medical and pharmacy teams to ensure that all medications are prescribed and dispensed prior to discharge.
• To liaise with the social services team, as directed, completing the notification to assess and discharge as appropriate.
• To organise patient transportation for discharge with family/carers, voluntary services or via Hospital transport.
• To undertake and/or assist with audits as required.
• To accurately document all communication with family, carers and external agencies in relation to the discharge process.
• To provide administrative support to ensure the smooth running of the Inpatient Unit, covering A/L, etc. as required.
• To record therapy sessions for patients on national audit systems.

Patient / Customer Care
• To answer the telephone in a professional manner and refer callers onto the appropriate member of staff as necessary.
• To ensure that the dignity and respect of the patient is maintained at all times, supporting the Trust’s values and behaviours.
• To maintain accurate documentation and databases.
• Support the scanning process using SystmOne. To update system one as required.
• Effectively communicate with patients and relatives/carers.
• To provide general, non-clinical advice, information and guidance to patients within the boundaries of the role.
• Regularly handles patient property, belongings and valuables.

ServiceDevelopment and Improvement
• To prepare and actively participate in the Trusts Annual Performance Development Review process and progress reviews.
• Ensure training and development needs detailed in your personal development plan are followed up and the effectiveness of the acquired training and development evaluated in term of self, patient and service.
• Suggest service development in reflection of data collected and user feedback received.
• To support the services in updating Trust compliance documentation.
• To comply with legislation, policies and procedures relating to own work area.
• To be an active participant of the ward team meetings and minute taking.

Communication and Relationship Building
• To give and receive information accurately, ensuring that patient confidentiality is maintained at all times.
• To communicate effectively with patients, carers, visitors and other staff members at all times.
• To understand and relay complex information regarding patients pathway.
• To produce written information that reflects the complexity of the role.
• To provide training of the role to other administrative staff including new starters.

Information Management
• To be responsible for the quality of data recorded. The data should be accurate, legible (if hand written), recorded in a timely manner, kept up to date and appropriately filed.
• To record information and scan documents onto SystmOne on behalf of the Unit.
• Develop, organise, and monitor information in a variety of formats
• Able to create and maintain documents and spread sheets
• Produce documents, charts, tables, databases etc. by use of Microsoft packages e.g. Word, Excel, Power Point, PowerPoint, Outlook and other software such as SystmOne.
