Key Clinical Responsibilities Working independently and alongside GPs to identify appropriate care paths and treatment methods for patients through face to face appointments, telephone consultations, and home visits Making professional, autonomous decisions in relation to presenting problems whether self-referred or referred from other health care professionals Using clinical nursing practice at an advanced level to deliver quality care and advice to patients and demonstrate an in depth knowledge of the options for achieving control of symptoms and assessing the health needs of patients with undifferentiated and undiagnosed problems Perform safe triage of patients when necessary via telephone, video, econsultation, and face-to-face consultations Prescribe medication appropriate to patient needs or refer the patient to a community pharmacy who will provide designated minor ailment services and prescription advice Provide information and advice on prescribed or over-the-counter medication on mediation regimens, side-effects, and interactions Support and educate patients to adopt and promote healthy lifestyles and apply principles of self-care Maintain a concept of nursing which incorporates physiological, social, and spiritual components Support the nursing team as Lead Nurse Manage Chronic Disease and QOF areas Therapeutic monitoring advice Home visits and housebound immunisation visits Path lab results 111 triage eConsultation triage Workflow, administrative, medication tasks In general, the post holder will be expected to undertake all of the normal duties and responsibilities associated with an Advanced Nurse Practitioner working within Primary Care. Communication Ensure that all written documentation is completed and that appropriate data is collected Promote and maintain effective liaison, communication, and relationships with all of the providers involved in caring for the patient Utilise and demonstrate sensitive communication styles to ensure patients fully understand advice and instructions Recognise people's needs for alternative methods of communication and respond accordingly Promote and practice excellent verbal communication skills with patients and colleagues Maintain effective communication within the project team and with external stakeholders, providers, and organisations as appropriate Communicate with, and support, patients with complex mental health issues Quality Recognise and work within own competence and professional code of conduct Produce accurate, contemporaneous and complete records of patient consultations to agreed standards Effectively manage own time, workload, and resources Support, and participate in, shared learning across the company and wider organisation In partnership with other clinical teams, collaborate on improving the quality of health care, responding to local initiatives as appropriate Evaluate patients' response to the consultation and the effectiveness of care Understand and apply legal processes for, and issues surrounding, the identification of vulnerable and abused children and adults, and be aware of the statutory child / vulnerable patient's health procedures and local guidance Health and Safety Use personal security systems within the workplace, according to practice guidelines Identify the risks involved in work activities and undertake such activities in a way that manages those risks Maintain awareness of local and national standards of infection control and cleanliness and regulatory / contractual / professional requirements and good practice guidelines Actively observe current working practices across the practice in relation to infection control, cleanliness, and related activities, ensuring that procedures are followed and weaknesses / training needs are identified and escalating issues as appropriate Maintain sterile environments and awareness of spillage control procedures Follow the correct and safe management of specimens process including collection, labelling, handling, use of correct and clean containers, storage, and transport arrangements Support Waste Management, including handling, segregation, container management, storage, and collection. Report potentially risks that have been identified Apply practice policies, standards, and guidance Team Working Understand your own role and scope, and identify how this may develop Work as an effective and responsible team member and be supportive to others Work effectively with others to clearly define values, direction, and policies impacting upon effective care delivery Discuss, highlight, and work with the team to create opportunities to improve patient care Learning and Development Assess your own learning needs and undertake learning as appropraite Participate in annual individual performance review including taking responsibility for maintaining a record of personal and professional development Disseminate learning and information gained to other team members in order to share good practice and inform others about current and future developments Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice Evaluate and review innovations and developments that are relevant to the area of work Enlist support and influence stakeholders and decision-makers in order to bring about new developments in the provision of services Managing Information Use technology and appropriate software as an aid to the implementation and monitoring of care, presenting, and communicating information Review and process data using accurate codes in order to ensure easy and accurate information retrieval for monitoring and audit purposes Manage information searches using the internet and local library databases Collate, analyse, and present clinical data and information to the team This job description may be reviews in the light of changing organisational and service needs.
