Job description
The post holder will assess, plan and evaluate the health needs of children looked after children in Swindon and the surrounding areas, challenging and escalating concerns where needed to ensure that the needs of the child are paramount. The post holder will work autonomously and in collaboration with colleagues across the childrens workforce to demonstrate improvements in health and wellbeing outcomes of children looked after by Swindon Borough Council. The post holder will ensure that the voice of the children clearly informs the delivery of services and their own practice. The post holder will support the gathering of information to inform the key performance indicators and local performance data on the health and wellbeing of children looked after and Care Experienced Young People. Ensure accurate and open communication and co-ordination with a range of organisations and individuals, researching and drafting correspondence and papers and ensuring the management of specific tasks, lead reporting and analysis across a range of specialties, functions and projects. The post holder will work with the Named Nurse for Children Looked After and the wider Children Looked After Health Team to share good practice and delivery of high quality health services, including statutory health assessments which identify and meet the health needs of children looked after. The post holder will act as a source of advice and support to Care Experienced Young People and the Positive Futures Team to support good health outcomes for this vulnerable group.
