Job description


This is an exciting opportunity to lead on the development of anintegratedNHS Talking Therapies and Long Term Physical Health condition service (NHS TT -LTC).

We are seeking a highly motivated and experienced Clinical or Counselling Psychologist, or Cognitive Behavioural Psychotherapist, who is accredited as a CBT practitioner by BABCP.

Applicants must have substantial experience of working as high intensity CBT therapist, Clinical or Counselling psychologist, in an NHS TalkingTherapiesservice, managing complex client presentations and providing formal clinical supervision to qualified staff and trainees. Applicants should also have experience of participating and leading in areas of service development such as service promotion and outreach, or perinatal, long term health conditions etc and have a passion and interest in promoting health and wellbeing within the wider community and engaging with relevant stakeholders.

Applicants must have good knowledge about the NHS Talking Therapies programme and have demonstrated high standards of communication skills, problem solving and have developed good professional working relationships with staff, patients, partners and the general public.

The post-holder will expand the existing care-pathways and case-finding systems with stakeholders to deliver the LTC NHS Talking Therapies integration. They will be responsible for data monitoring and reporting systems against Key Performance Indicators, with support from the business manager.

They will use patient level data, patient feedback, complaints and incidents information from the LTC team and wider service to evaluate and report on the efficacy and performance of the LTC integrated team, communicating at a high level via written reports and presentations.

The post-holder will be working from an established base with experienced staff working within the integrated Greenwich LTC service. They will be part of the senior staff group, and will contribute to the overall management and governance of the service. The post-holder will be responsible for supervising and line managing CBT therapists and directing the work of the psychological well-being practitioner LTC staff (with the PWP team lead).

The post holder will be resilient, with a ‘can do’ attitude and have the skills, experience and personal qualities required to be able to manage the competing demands of building and maintaining the morale of staff whilst meeting the service’s targets for quality and activity.

The post holder will receive regular clinical and managerial supervision and support and join other senior clinicians within the service.

Oxleas offers a wide range of NHS healthcare services to people in community and secure environment settings. Our services include community health care such as district nursing and speech and language therapy, care for people with learning disabilities and mental health care such as psychiatry, nursing and therapies. Key Task and Responsibilities
• To lead on the development and implementation of the integrated Long Term Condition NHS Talking Therapies team in line with national guidance.
• To be responsible for expanding on the care-pathways and case-finding systems with stakeholders to deliver the LTC NHS Talking Therapies integration.
• To research, devise and implement LTC specific interventions, utilising NICE recommendations and other published LTC resources for the client group.
• To develop expertise in working with long-term conditions within the service and work within the framework of NHS Talking Therapies national requirements to provide specialist supervision across staff groups, as necessary
• To be responsible for data monitoring and reporting systems against Key Performance Indicators
• To use data and information from the LTC team and wider service to evaluate and report on the efficacy of the service, communicating at a high level via written reports and presentations.
• The provision of highly specialised CBT to service users experiencing PTSD, OCD, Depression, Anxiety and other mental health problems.
• The provision of CBT supervision for High and Low Intensity trainees and qualified Specialist Psychological therapists and other psychological therapists and students.
• The personal provision of consultancy, support, professional advice and clinical leadership to carers, other professions, and other agencies providing support for the client.
• Maintenance of professional standards, codes of conduct and good clinical governance.
• To work effectively with the Clinical Lead to promote the Greenwich Time To Talk service.
• Good relationships with partner organizations including voluntary, non-statutory and private sector organisations.
• Advises service management about psychological approaches and needs for service users.
• Effective liaison and supervision for other staff who may in the future join the service.
• To maintain professional accreditation with relevant professional body eg the BABCP, keep an updated CPD and reflective practice log and attend training to maintain skills and competencies as appropriate.

Management responsibilities
• To plan and prioritise own caseload and time management.
• To implement and maintain the Trusts’ policies and procedures
• To participate as a senior clinician in the development of the service where

Clinical or operational matters need addressing.
• To line manage and performance manage junior staff.

Leadership
• To lead on the development and implementation of the integrated Long Term Condition Improving Access to Psychological Therapies (LTC NHS Talking Therapies) team in line with national guidance.
• To provide high quality CBT supervision for High and Low intensity trainees and qualified staff engaged in CBT.
• To provide courses/seminars/workshops/presentations relevant to specialist area including teaching on courses in CBT and Step 2 and 3 interventions.
• To actively contribute to relevant committees/projects/working groups of the service as required,
• To lead on specific projects or areas of responsibility.
• To represent the Service with other agencies and organisations.

Clinical
• To exercise professional responsibility for the assessment, intervention and discharge of clients, using highest level of interpersonal and communication skills when presenting and receiving complex, sensitive and contentious information to clients, family, support workers and other professionals
• To provide highly specialised psychological therapy, collaboratively with clients

including:
1. Assessments: Psychometric assessments including the full range of NHS Talking Therapies data set requirements making use of Data management systems; behavioural assessment; observational assessment; assessment by interviewing; assessment by analysis of archive material, multi-disciplinary notes and discussions, reviews, and supervision.
2. Interventions: Behavioural therapies and Cognitive Behavioural Therapies and other psychological therapies as appropriate;
3. With a range of presenting problems primarily including Anxiety, Depression, PTSD, OCD but including the full range of primary care mental health problems, alone and in the context of long term health conditions.
4. To develop, monitor and support interventions that will be applied by Psychological Wellbeing Practitioners.
• To provide highly specialised input to service development of CBT, counselling and evidenced based therapy within Greenwich.
• To provide guidance and advice to relevant significant others (such as parents, partners or peers) in order to improve the psychological health of clients.
• To provide advice to other professionals within and external to the multi-disciplinary team, and agencies that provide services to this client group
• To keep adequate clinical records in accordance with Trust guidelines, the
• BABCP/UKCP or BPS Division of Clinical Psychology and DoH guidance
• To frequently sit in a restricted position for substantial proportion of working time during clinical sessions and meetings, requiring long periods of concentration
• Dealing with frequent highly distressing situations i.e. clients with severely distressing mental health experiences, past and current abuse. Dealing with unpleasant working conditions i.e. verbal aggression, physical threats from client, family or others.

Clinical Governance
• To implement and maintain the principles of Clinical Governance, including risk management.
• To implement and maintain good information governance in respect of British Psychological Society’s (or other relevant professional bodies) and Department of Health’s codes and guidance’s on the proper management of client information, the Caldicott principles and the Data Protection Act
• To abide by the post holder's appropriate Code of Professional Conduct and to follow guidance from NICE and the Department of Health.
• To engage in ongoing personal professional development.

Research and service evaluation
• To use published research and information on good practice regularly to keep clinical practice up-to-date and evidence based.
• To participate and lead service evaluation and audit.
• To use research evidence, service evaluation and audit to inform service developments within the service.

Teaching and Training
• To provide educational inputs on CBT to other professionals and to staff of other agencies and organisations as required.
• To participate in staff recruitment for various positions within the service as and when appropriate
• Proposing and undertaking research relevant to the client group.

Communication
• Clinicians within Greenwich Time To Talk service and clinicians within Oxleas NHS Foundation Trust.
• Senior Trust Officials, when indicated or appropriate i.e. service development
• General Practitioners and GPs with special interest.
• Service providers from both statutory and voluntary organisations e.g. Adult Mental Health Services, CAMHS, Social Services, Mind and GLLAB.

On Call/Unsocial Hours
• To participate in out-of-hours clinical sessions (one evening per week) under Agenda for Change terms and conditions. This will include Saturdays, if this becomes part of the service provision.

Greenwich Time To Talk is a large and well established service (since 2009), and is responsible for the provision of Low intensity and High Intensity therapies, including brief counselling within the standards of stepped care model at steps 2 and 3. The staff team provide services across the borough of Greenwich in GP practices, healthcare centres, community organisations as well as the main team base in Eltham.

The service is well subscribed to and has good systems in place to manage incoming referrals and waiting lists. National and CCG targets on waiting times and recovery have been met regularly and consistently. The team are friendly, welcoming and have excellent systems in place for peer support. Staff have regular clinical supervision. Training and development are emphasised, and staff wellbeing and team working are given high priority.
