Job description

Aspects of the role include To assess, plan, implement, promote and evaluate safe, creative and effective treatment plans in collaboration with the patients diverse needs, their carers and other professionalsDeliver interventions that are evidenced basedBe responsible for the safe monitoring, management and administration of medicationWork autonomously, building leadership skills and advanced clinical assessment skillsYou will build effective relationships with patients, their carers, family and colleagues across the MDTYou will develop exceptional planning and organisational skillsYou will be able to analyse care and treatment provided, and develop sound clinical decision makingYou will be trained to deliver high quality care and develop Knowledge in all areas including palliative care, leg ulcer management, acute and chronic wound care, understand bladder and bowel dysfunctions, provide supportive care to carers and family membersYou will delegate and support junior staff, building leadership skills A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
