Job description
The post holder will operate within the Frailty and Palliative Care Team (FPT), orchestrating patient care throughout the organisation under the guidance of the Manager. This role entails bolstering the FPT by offering additional time, resources, and expertise to assist patients in readying for or following up on discussions with primary healthcare professionals. You will take charge of organising, setting up, and documenting multidisciplinary team meetings (MDTs), making sure all tasks are carried out. Additionally, you will oversee the coordination, preparation, and documentation of support for patients in care homes, including both virtual and in-person ward rounds.
