Job Title: Advanced Clinical Practitioners, Treatment Centre, Bournemouth East Primary Care Network
Contract Type: Permanent

Hours: up to full time (37.5 hours per week), Monday to Thursday and Saturdays.

Salary: Up to £25.90 per hour (In-hours), £33.67 per hour (OOH – 6.30pm-8pm and Saturdays)

Location: PCN Treatment Centre based at Shelley Manor Medical Centre, Bournemouth

Bournemouth East Collaborative Primary Care Network (PCN), situated on the stunning Dorset South Coast, comprises four like-minded practices working together in East Bournemouth, with a strong reputation on quality improvement and investing in its employees. The PCN serves a population of approximately 55,000 patients with a diverse demographic.

The Role

Bournemouth East Collaborative Primary Care Network (BEC PCN) has a new and exciting opportunity for an Advanced Clinical Practitioners to join the BEC Treatment Centre Team .

We are looking for Advanced Clinical Practitioners to work in hours and out of hours service at the PCN Treatment Centre. The service runs Monday to Thursday 4:00pm to 8:00pm, plus Saturdays 9:00am to 5:00pm.

This role will require an Enhanced DBS Clearance . Should you have this certificate as part of the DBS subscription service, we would be happy to accept this providing the original is shown. Otherwise, you will be expected to complete a new application.

Job Summary:

The Treatment Centre is based at Shelley Manor Medical Centre, comprising of multi-disciplinary healthcare professionals working towards a vision of providing integrated care for all our patients and improving access.

Advanced Clinical Practitioners will be expected to provide assessment, diagnosis, and treatment to same day face to face appointments. If appropriate prescribe and refer patients to appropriate care providers, within locally agreed PCN healthcare pathways and complete 2 week waits if required.

Main duties of the job

Autonomously manage own appointment list of patients across the age continuum to provide whole episode of healthcare by seeing, assessing, diagnosing, treating and discharging or referring patients attending the Treatment Centre across a wide range of undifferentiated and undiagnosed conditions including minor injuries and minor ailments.

Able to use an advanced level of professional accountability, autonomy and judgement for seeing treating and assessing patients who present with a broad range of situations and conditions.

You will have an advanced practice qualification and be a non-medical prescriber.

Plan and manage complete episodes of care, working in partnership with other members of the multi-disciplinary PCN team, delegating and referring as appropriate, to optimise health outcomes for patients.

Work autonomously, clinically examining and assessing patients and initiating any appropriate action whilst taking responsibility for managing practice whilst using an advanced level of professional accountability, autonomy and judgment for highly complex decision making across a broad range of situations.

Have a level of understanding of PCN and NHS values, policies and procedures.
