Kinson Road Medical Centre JOB DESCRIPTION Job Title: Advanced Practitioner Hours: 30 (Tue to Fri) Responsible to: Senior GP Partner and Practice Manager Responsible for: Providing a high standard of patient care, as part of a dedicated team, in an efficient and confidential manner whilst working within professional boundaries and qualifications. Job Summary: The Postholder will be required to work as part of the practice team, helping with on the day requests which will include online & telephone triage, face-to-face consultations, daily triage clinics and visiting patients in their home. The Postholder will provide care for the patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.As part of a small but effective on the day within the practice, the Postholder will work directly alongside 1 Advanced Nurse Practitioner and dedicated GP to make referrals and to meet the needs of patients effectively and efficiently.Core tasks: 1. Clinical To assist in the online and telephone triage of on the day patient requests. To work autonomously, but with supervision from a GP, to advise on or signpost patients to self-care, to arrange home visits, or to arrange for face to face consultations to either see directly or another member of the clinical team. To assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis on an acute basis within the boundaries of competence. To clinically examine and assess patient needs from a physiological and psychological perspective, interpret test results, and plan clinical care accordingly. To arrange investigations as appropriate with other members of the practice team or with acute care and community services as appropriate. To work within the clinical team to treat patient needs in accordance with evidence-based practice and national and practice protocols within level of competency, seeking the advice of the duty doctor or other staff if outside level of knowledge. To discuss with the duty, doctor the needs of patients presenting with complex health needs or conditions requiring treatment, hospital admission or referral. To liaise with secondary care and community teams as appropriate to ensure the immediate care and treatment needs of patients are met as appropriate. To undertake home visits as assessed by the duty doctor within the boundaries of professional qualifications. To manage inbound administration (post/electronic communications) to ensure any medical intervention required is referred to duty GP for appropriate action. 2. Teamwork To work autonomously, as part of a busy team to manage and triage requests for on the day services, whilst also being flexible to support the wider practice team. To act as a clinical role-model/advocate, working alongside the practice team to develop and deliver care that is responsive to changing requirements. To support and educate patients in health wellbeing and prevention. To work with the practice team to drive forward changes and to lead service redesign in establishing the duty team as an integral part of managing patient care, taking account of both the administrative and clinical changes required. To be an active and integral member of the practice team, attending meetings and training sessions, as required. To contribute to policy development and implementation of procedures and guidelines in relation to own area of work and that of the duty team. To assist in maintaining a safe environment for the duty team, patients, and co-workers, ensuring that all statutory and local regulations (e.g. Health & Safety, COSHH, clinical waste management, infection control) are implemented and followed 3. Administration To ensure data is recorded accurately and timely onto patient records using SystmOne and other IT software, with due regard for record management and patient confidentiality, storing information securely in accordance with protocols. To ensure data is accurately recorded and coded onto relevant templates and documentation associated with patient health care. To collect data and participate in regular Practice audits and research projects. To follow the principles of Safeguarding Vulnerable Adults and Children in accordance with agreed policies and procedures. To support the equality, diversity and rights of patients, carers and colleagues according to Practice protocol. 4. Professional Development To be responsible for continued professional development, assessing own learning needs and attending study days and courses as appropriate. To ensure mandatory staff training is completed annually or as appropriate. To participate in an annual appraisal. To be aware of quality standards and targets as set out in NICE guidelines, NHS contracts, Quality and Outcomes Framework, etc., working to achieve these standards always. To promote a professional approach always by attire and attitude to patients and staff both in and out of the Practice. To maintain annual registration with the relevant professional bodies (as required) and to adhere to any associated code of professional conduct. The post holder will also take on any other reasonable duties which may be requested from time to time by the GP Partners or Practice Management. The above represent the main duties of the post and may be subject to change, to meet the needs of the Practice and patients. Any changes will be discussed with the post holder, where possible prior to implementation.
