Job Description - Support Worker
Pertemps Exeter are proud to be working with a charity organisation who are currently hiring for a Support Worker within their outreach team to work alongside their team based in the centre of Exeter.
Salary of £23,240 annum
This is a fixed 3-month contract, full-time role working 37.5 hours per week. The role is located in Exeter however due to the nature of the role a valid driver's licence and access to a car for business purposes would be required.
Participation in an on-call rota, 1 in every 4 weeks, £100/week.
Responsibilities include:
Complete detailed and thorough sign-ups to ensure that new clients moving into the service understand their rights and responsibilities and the conditions of their license agreement and have a valid Housing Benefit claim in place.
Ensure new clients are welcomed and inducted into the service and are familiar with the area, local facilities and other services.
Assess, plan, deliver and review person-centred support plans which empower clients to progress towards their goals and greater independence.
Empower clients to engage effectively with other professionals such as community mental health teams, social work teams, housing providers, health professionals, probation workers, Job Centre Plus.
Prioritise safeguarding and promote the welfare of adults, children and young people who are potentially at risk. Undertake internal and external safeguarding training to ensure that safeguarding is everyone's priority.
