Post -PCN Care Coordinator Salary Grade -£22,732 Reporting to -Service Lead / Manager PURPOSE OF THE ROLE The post holder will work closely with teams to assist in co-ordinating care utilising population health intelligence. The role will promote integrated working across Rushcliffe Primary care network (PCN), General Practice and existing statutory and voluntary health and social care services. The role supports a variety of services including, but not exclusive to; mental health, social prescribing, pharmacy, respiratory, vaccination, care homes, extended hours and other services identified as a priority within the Primary care network (PCN). MAIN DUTIES AND RESPONSIBILITIES Understand what a primary care network is, and how support is provided to patients as a result of improved collaboration of working between health and social care services. Utilise data to help identify and work with groups of patients to deliver personalised care and achieve Investment Impact Fund (IIF) indicators. Record patient interventions / contacts on relevant electronic database systems within agreed time scales (e.g. SystmOne / Pinnacle). Generate reports and analyse data using electronic clinical systems such as eHealthscope and SystmOne Support patients to book appointments, as part of the various projects, programmes and clinical initiatives (e.g Spirometry, Mental Health and extended access/community clinic appointments). Help patients manage their needs by answering queries, making and managing appointments, and making sure that patients have understandable written or verbal information to help them make choices about their care. Support or manage clinics as required including management and monitoring of extended access and community staff rotas. Support the delivery of Help patients gain access to self-management education courses and peer support/interventions that support them to take more control of their health and wellbeing Provide coordination and navigation for patients and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care staff. Support the coordination and delivery of mutli-disciplinary meetings within the PCN. Work with GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN Raise awareness within the PCN of shared decision-making and decision support tools Identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared to have shared decision-making conversations. Provide coordination of, support and participate in relevant internal and external working groups and provide project support when requested Work with key people in the PCN to develop & support collective general practice projects including areas of federated working Work closely and in partnership with the Social Prescribing Link Workers. Adhere to organizations policies and procedures, guided by occupational policies and procedures in primary and secondary care. Maintain up to date skills and knowledge and maintain awareness of professional issues. Work without supervision, plan own workload and seek guidance as required from line manager and colleagues. The post holder should demonstrate good organization and time management skills Use clinical systems for record keeping, audit and evaluation Develop and implement data collection systems that will provide accurate and timely data. Administrative support for clinicians. Maintain confidentiality at all times. LEARNING & DEVELOPMENT Ensure continuing education, training and development is undertaken to meet role specific and governance guidelines for Continuing Professional Development. Continue to develop soft skills and emotional intelligence in line with the requirements and culture and to uphold the values and culture of PartnersHealth. Undertake ongoing training as required for the role of the care co-ordinator.
