Supporting the citizens of Powys to achieve their potential and access services that will improve their quality of life. Working under The Wellbeing Act (Wales) 2014 in building local communities resilience.

What you will do:

• The role includes reviewing current assesment and care plans
• Recording information electronically,
• making appropriate referrals to partner agencies,
• multi-disciplinary working,
• opportunity for continued professional development.
