Job description
Actively promote a person centred, Strengths based ethos through partnership working Complete assessments and reviews that will identify risks, needs, options, and promote well-being and maximise independence Make decisions in a timely and effective way using sound knowledge of legal frameworks, HCC policy and processes to ensure seamless transition into onward services Provide correct, timely and robust information to service users, families, in-houses services, and partners Manage expectations of service users, other professionals, family and friends Build knowledge of how the community reablement service works and share with others Support professional practitioners, under their direction, in the assessment, planning and review of safeguarding cases Understand demand and manage capacity effectively through joint forward planning Support professional practitioners, under their direction, in the management of high- risk cases and/or situations, and ensure social care interventions are proportionate to need Management and prioritisation of workload, know when to seek advice and/or escalate Respond appropriately to concerns and complaints and follow appropriate escalation routes Carry out assessment and prescription of standard equipment and adaptations, seeking support from seniors where needed Provide information and advice to local community networks, to support local empowerment, well-being and maximise independence. Ensure the staff and/or services you manage/supervise provide the same Advocate on behalf of individuals and communities in the maintenance of their independence and wellbeing, and seek support and advice when this is challenged Build and maintain working relationships with multidisciplinary staff Work with multi-disciplinary teams in the management of high-risk cases, and support practitioners in high risk multi-disciplinary case work Adhere to protocols to share information with multi-disciplinary teams, and support the staff you manage/supervise to do the same Identify cases that will benefit from planned multi-disciplinary working, and/or an appropriate response to unplanned work
