Job description
We require Social Workers (you must be a qualified Social Worker to apply for this role)
To join The High Needs Team - as the Supported living social worker - based at the guildhall in Hull who are making a daily difference to the lives of the people we serve. If you are passionate about making a positive and lasting difference for the people of Hull, we would like to hear from you!
You will join an integrated multi-disciplinary team who provides consistent and intensive case management to promote independence and give Autonomy to enable people to live the lives they choose to lead, whatever age or stage of their life.
You will provide high quality social work practice, ensuring individuals have a solution focussed plan towards achieving their goals and maintaining their wellbeing. You will work creatively with individuals, local communities and our partners, empowering them to deliver the best possible outcomes for people and to create a positive Adult Social Care culture.
You will have oversight across packages of support which may involve a deprivation of liberty and where there is a requirement to regularly review the restrictions which are in place to keep the person safe, with a that support can be reduced over a period of time.
You will be working with Multi agency professionals, mental health, learning disability teams Childrens social workers and disabilities team as part of the transitioning young people into adults services that are going to be living in supported living……to plan, prepare and deliver bespoke services for people with complex needs. You will ensure through ongoing case management that these services continue to meet needs and are best value.
We are looking for individuals who can demonstrate:
You will hold a professional social work qualification registered to Social Work England and will have experience of assessment and planning, initiating, monitoring, developing and providing outcomes-driven services in adult social care.
As a valued member of the team, you will have access to peer support, 1:1 time, supervision and ongoing training & development opportunities and to work with our partners across Social Care, Health, Housing and Voluntary Services.
