Job description
The Response Hub is based in the Rochdale, Heywood and Middleton area providing assessment, triage, signposting and support to individuals struggling with their mental health.
The Response Hub consists of several services including:
• Assessment and triage service
• Police Liaison
• Criminal Justice
• Joint Response Vehicle
• Out of Hours Team
• Primary Care Practitioners (PCNs)
• Trainee Associate Psychological Practitioners (TAPPS)
• Consultant Psychiatrist Clinic
The position we have available is within our Assessment and Triage service. The role involves undertaking comprehensive assessments and providing expert advice, education and support to individuals. You will provide guidance in risk management and help signpost and refer on to relevant services where required. You will have the support of the MDT and the consultant psychiatrist to formulate a robust management plan.
The Response Hub is developing into Living Well working towards providing a more local service to individuals in their local communities, the role will involve working together with partners including MIND to provide a collaborative and supportive approach to care and support in the community.
The service covers 7 days a week, inc bank holidays. This position is for the day position only, hours of 8am-9pm (no night cover required). The Response Hub is committed to providing a supportive working environment therefore flexi working applications are considered.
• The post will involve triaging referrals and assessing the appropriateness for mental health support, and signposting to other services if not appropriate.
• This post will be responsible for the assessment and formulation of presenting mental health problems experienced by individuals at a secondary care level.
• You will also be responsible for delivering brief psychological based interventions for people experiencing common mental health problems.
• Implement recovery pathways
• This post involves liaison with other roles and services within the community.
Rochdale Response Hub offers advice, signposting and support to individuals who require support with their mental health.
The service is the central point of entry into mental health services for Heywood, Middleton and Rochdale residents over the age of 18. The team receives routine and urgent referrals into mental health services, they also accept diverted emergency assessments following attendance at A&E.
