JOB OVERVIEW
An exciting opportunity has arisen to join the Dementia Care Team as a
Band 5 Nurse. You will be working across both sites, The Royal
Shrewsbury Hospital and The Princess Royal Hospital, Telford,
assisting patients living with dementia, their families and supporting
staff affected by dementia.
The successful applicant will be be responsible for establishing this
new role, working across the organisation and professional boundaries
to bring about improvement in the care and treatment outcomes of
patients with dementia by developing a more integrated, and less
fragmented, approach to care.
MAIN DUTIES OF THE JOB
To take an active role in developing care services to patients with
Dementia across the organisation, striving to improve, maintain and
monitor the quality of Dementia care through audit of practice,
surveillance, education.
WORKING FOR YOUR ORGANISATION
The Dementia Care Team are a small but friendly team who work cross
site to enhance the knowledge and skills of the teams across the
organisation to improve patient care experience.
