Job overview

Following a review of inpatient discharge processes early in 2022 it became evident that additional support to encourage the early planning, comprehensive assessment and support towards discharge of those admitted to a Trust hospital was necessary. It was also evident that a strong emphasis on the individual’s social and home circumstances was required from a professional social work perspective.

We are looking for a dynamic social work practitioner who can bring an expertise in social work to a multidisciplinary inpatient setting. A strong background in mental health social work, related legislation and community support should combine with the ability to drive an inclusive strengths based approach to effective discharge planning.

Main duties of the job

To actively support the provision of timely discharge of patients from the mental health inpatient units within the Trust, which serves people from across the Lancashire & South Cumbria footprint.

This pilot work can be based in any of the major hospital locations by agreement.

The post is a temporary position for 12 months as part of ICB funded project to assess the benefits and advantages of direct inpatient social work intervention and support during the admission of a patient. This is in response to recent surveys of practice and service delays related to discharge processes and is intended to strengthen and add momentum to the process of discharge planning, enhancing the inpatient service response and the multidisciplinary and multi-organisational provision.

The Post Holder will work in close partnership with a range of professionals, agencies and organisations both internal and external to LSCFT to ensure effective discharge processes are established and implemented.

The Post Holder will work in close partnership with a range of agencies and organisations internal and external to LSCFT to ensure robust and streamline inpatient care pathways, take a lead role in facilitating discharge planning which will include early identification of barriers to discharge, taking action to prevent any unnecessary delay to discharges, facilitating complex multi-agency case discussions in relation to discharge.
