Job description
Under the supervision of the Home Health Director, is accountable for effectively triaging calls during evening/night shift. These responsibilities will be carried out in accordance with organization policies/procedure and standards consistent with Federal, State and accreditation regulations.
Duties and Responsibilities:
Demonstrates effective communication and education of family/caregivers, and referral to appropriate disciplines
Assess and triage the patients, identifying needs, issues, resources and targeting outcomes via the electronic communication.
Refers cases to Ethics Committee or State agencies for consultation as needed.
Informs patient/family members on method of communicating satisfaction or dissatisfaction with care provided.
Guides, directs, and deploys night on-call staff.
Maintains appropriate documentation of calls and beeper logs, included case communication notes in the Electronic Medical Record.
Complies with all State and regulatory standards for timeliness and completeness.
Demonstrates flexibility and creativity in identifying resources to meet patient needs.
Effectively communicates with staff, managers, and patients.
Participates in the development of procedures, roles systems and structures.
Responsible for triaging patient phone calls, deploying the on-call nurse, and communicating to the management team.
Other duties may be assigned.
Requirements:
RN license required and managerial experience.
Minimum of two years clinical nursing experience in a medical/surgical or acute care environment.
Minimum of one year home care experience preferred.
Graduate of an accredited professional nursing program required. Bachelor’s degree in Nursing preferred.
