About the role
- Promoting client participation and choice through the provision of effective, responsive, goal focused and outcomes-based support to Home Care Package clients.
- Working with clients from diverse backgrounds and their informal networks to ensure their individual needs are identified and supported.
- Looking after a caseload of elderly clients who wish to remain living independently in the community, assisting them to achieve their independence goals and use their HCP funding to its full potential by accessing care services, allied health, social support and nursing services.
- Engaging new clients, completing intake assessments and establishing client centred support plans.
- Working collaboratively within a multidisciplinary team to ensure outcomes are achieved in line with client goal directed care plans, and service objectives.
- Conduct regular quality assurance visits with clients in person and/or over the phone to ensure the highest level of care is being achieved while also identifying any changing needs or requirements for additional services.
About you:
- Tertiary qualifications in Nursing or Allied Health discipline
- Experience with Case Management in particular managing a caseload of clients from goal directed care principles
- Preferably have qualifications and experience in care planning, identifying needs, goal setting, coordinating and managing care services within the aged care and community health
- Financial skills to effectively develop and monitor package expenditure for Home Care Package recipients.
- Demonstrated understanding of Aged Care Quality Standards and Home Care Program guidelines
- Strong attention to detail
- Excellent verbal and written communication skills
- Able to work independently and as part of a multidisciplinary team
- Need to have a driver’s licence and working rights in Australia
