Hours per week: 35-37 Monday – Friday, normal working hours

This opening assignment is for 5-6 months

City: MIDDLESBROUGH

The purpose of this role is to lead on the council’s operational approach to the management of NHS Continuing Care for Children, and joint funding agreements to ensure compliance with national guidance. To provide professional expertise to social care colleagues regarding the continuing care process (CCC) for children, and to act as a specialist resource for advice and expertise to colleagues with respect to this. The role is to lead, model and develop excellent social work practice across Children’s services with the emphasis on partnership work with colleagues from Health.

Key responsibilities:

Implement the principles and processes outlined in the National Framework for NHS continuing care.
Provide professional expertise to social care colleagues regarding continuing care process (CCC) and to act as a specialist resource for advice and expertise to colleagues with respect to this.
Responsible for ensuring team managers, social workers and other frontline practitioners make efficient and effective use of the resources, ensuring appropriate referrals are made for consideration of CCC under the National framework.
Support service managers in progressing health financial support and to take a key role in funding/joint panels.
Develop, monitor and evaluate the implementation of policies, procedures and practice guidance, relating to Continuing Care both internally and with partner external agencies.
Undertake a lead role in defined projects to further health and social care integration and be responsible for transient line management arrangements in relation to these key project lead responsibilities.
Responsible for influencing social care staff to change practice by developing new ways of working to improve outcomes for Children’s by improving systems and processes that will support social care staff and their practice.
Take a lead role in supporting the Council in its commissioning, monitoring and contractual compliance of packages of care through active intervention to prevent poor practice arising and support in the promotion of quality and good practice.
Develop and sustain positive working relationships with partner agencies to support integrated multi-agency packages of care and increase understanding of complex needs.
Maintain a good knowledge of emerging policies from government departments.

Qualifications:

BA SW, DIP SW or equivalent professional social work qualification.
Evidence of social work specific post qualification training.
Practice Educator Qualification or equivalent teaching/coaching qualification.
Management qualification desirable.

Experience:

Significant social work experience of working within Children’s social care. Substantial case management experience of working with children within Child Protection, Court and other formal processes.
Experience of delivering social work training, coaching and
Experience of collaborating and partnering with both external partners and internal teams and services.
Previous experience of supervision or management.

Knowledge:

Knowledge and experience of the legal framework in children’s care.
Knowledge and experience of undertaking reflective supervision.
Excellent working knowledge of Children’s Continuing Care and Continuing Health Care Framework.
Excellent working knowledge of Mental Health and Mental Capacity legislation.
Excellent working knowledge of the Data Protection Act 2018 (GDPR) and dealing with Subject Access Requests.

Skills & Abilities:

Ability to critically analyse and synthesise complex legislative, policy, research and service documents.
Ability to deliver at pace.
Ability to make effective decisions.
Strong IT skills including Microsoft packages or equivalent (e.g. G Suite).
