Job description
The Health Care assistant will function as part of the multi-disciplinary team within the ward or department to provide a seamless service & high-quality care to patients under the direct supervision of a Registered Nurse. The HCA is expected to undertake care to meet the needs of the patients under the supervision of a qualified nurse, working within the Trust’s policies and procedures and the philosophy of the ward. Working collaboratively as part of the ward team.
It is expected that the Health Care Assistant will continue to develop their own capabilities
and knowledge of nursing tasks in line with the Trust’s career pathway
Clinical Responsibilities
At all times maintain the dignity and respect of patients
Maintain excellent communication with patients, relatives and members of the MDT regarding all aspects of care demonstrating a variety of communications skills in accordance with the client group
Implement patient care plans as instructed by a Registered Nurse
Assist in the admission process by accurately recording patient’s demographic details
To assist patients with personal and oral hygiene as instructed
To assist patients with eating and drinking
Monitoring and accurate recording of dietary and fluid intake appropriately reporting any abnormalities to the nurse in charge
Enable patients to access appropriate toilet facilities, collecting and disposing of body waste
Testing blood sugars, urine and stool specimens & accurately record results
Removal of male/female indwelling urethral catheters
Obtain specimens & process specimens to the laboratory
Provide pressure area care to patients
Provide care to patients who are dying
Inspect & accurately record the condition of patient’s skin integrity, appropriately
escalating concerns
Able to prepare equipment for aseptic procedures as instructed
Taking and recording of temperature, pulse, respirations, blood pressure, oxygen saturation and pain scores on instruction from the Registered Nurse.
Accurately recording the results and reporting significant findings

Make a thorough assessment of the patient’s needs and document these in accordance with Trust policies.
Formulate a plan of care based on the identified needs of the patient, taking into account current
evidence based research findings.
Ensure that all the care planned is delivered to the patient at the highest possible standard.
Evaluate and document nursing actions and alter the care plan in accordance with the changing
needs of the patient.
To act as an advocate for the patient at all times and promote patient health through education
Pro-actively communicate and liaise with all members of the multidisciplinary team on matters
affecting patient care.
Ensure the rights of patients are respected at all times and assist in the provision of information,
enabling informed decisions to be made about their care.
To commence discharge planning on admission, co-ordinate through to the safe discharge of
patients from hospital, as per Southend University Hospital NHS Trust Discharge Policy.
In conjunction with the Ward Manager/Deputy Ward Manager, identify and plan appropriate
interventions that contribute to high staff morale and good working relationships with all members
of the multidisciplinary team.
Positively contribute to the process of change within the ward.
Contribute to the reduction of healthcare acquired infection by ensuring a high standard of infection
control practice in the light of research, current best evidence and legislation.
Adhere to the Trust Policies and Guidelines for the Safe Handling, Administration, Storage and Custody of Medicinal Products.
