Job description
Participate in the ward based Medicines Management (MM) service to medical and surgical wards within the trust to ensure all patients receive the best care regarding the management of their medication. Participate in the development of MM services within the trust in align with good clinical practice. Order and supply medication to the individual patients as appropriate to ensure the patients receives all required medication in a timely manner. To provide a technical role alongside the clinical pharmacist and other healthcare professionals in a ward based environment to support patients in the management of their medication to include: Medicines Reconciliation. Assessing patients own drugs. Transcribing medication for the individual patients. Checking medication on discharge. Counselling patients on their medication.
