Advanced Nurse/Clinical Practitioner

Opportunity available for a Advanced Nurse/ Clinical Practitioner to work within Intermediate Care Team, Based Near Leeds

The team sits within the Intermediate Care Team with the role predominantly acting within their professional boundaries, will provide care for the presenting patient, from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. As part of this role you will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for those resident in care homes and patients in their own homes. The post holder will also be expected to support in an acute environment when required to facilitate discharge. You will within Short Term Assessment Response Team to meet the needs of patients, supporting delivery of policy and procedures. In order to work at this level, NMC and Health and Care Professions Council (HCPC) requirements for advanced practice must be met.

The hours for this role are Full Time working 3 x Long Days Per week including 1 in 4 Weekends.

The Hourly Rate for this role:
Monday to Friday: £36.00
Saturday: £45.00
Sunday: £54.00
