Ref: S11135 | Stroke Association Support Coordinator | Homebased, Hywel Dda Health Board area, Wales. However, extensive travel will be required as part of this role and applicants are required to live in the locality to which they are applying. (May include team meetings or other work related meetings)
Salary circa £ 18,277 per annum | 25 hours per week
Our services are contracted, we currently have funding for this contract until 31st March 2027.
When stroke strikes, part of your brain shuts down. And so does a part of you. That’s because a stroke happens in the brain, the control centre for who we are and what we can do. It happens every five minutes in the UK and changes lives instantly. Recovery is tough, but with the right specialist support and a ton of courage and determination, the brain can adapt.

We provide specialist support, fund critical research and campaign to make sure people affected by stroke get the very best care and support to rebuild their lives.

We are looking for an enthusiastic and motivated individual to join the Stroke recovery team based in the Hywel Dda health board area.
Reporting to the Stroke Delivery Coach, the Stroke Association Support Coordinator will support stroke survivors and their families and carers by assessing the needs of each stroke survivor and their family.

This role requires extensive travel across a large geographical locality to visit people at home and in community settings. The use of a car with business use cover and the ability to drive is essential to be able to fulfil the requirements of the role.
To fulfil the role you must be resident in the UK and have the right to work in the UK.
