Salary: NJC Grade 10-11 depending on experience and qualifications: £37,336 - £44,428
Working Pattern: Part time (22 hours/week) & full time considered

The Transition Teams are county-wide teams that offer a service to young people from the age of 16 who have lifelong disabilities. Lifelong disability transition services are for those young people with physical disability, learning disability and/or autism. The focus of the team is to support people moving from childhood to adulthood and plan for the time after the person turns 18.
Transition is a busy and exciting time for workers and young people. You will be expected to work within organisational, policy and legislative frameworks as you support young people to increase their independence and plan for the future.
As a social worker you will be responsible for achieving positive change and improved outcomes for young people. You will have a varied caseload that can be complex and challenging and you will provide an accountable social care service to those people who access or need the service. You will have the opportunity to apply your social work skills, experience, and values to the work that you do in supporting young people, their support networks and your team.
You will be expected to work within organisational, policy and legislative requirements which include the Care Act, Mental Capacity Act and the Mental Health Act. Good working knowledge of safeguarding, risk assessment and enablement is essential.
A person-centred approach to transition planning is necessary to help young people and their families prepare for the journey into adulthood, so an ability to work closely with families and young people and to promote their views as well as their wellbeing in all interventions. This will involve working alongside our colleagues in Children's Social Care, Leaving Care, Special Educational Needs and Health. Knowledge of support and services available from education, health, housing, employment, or further education is desirable. We are seeking someone who is able to work innovatively, demonstrating a good understanding of Community Led Support as a model for delivering services.
You will work collaboratively with individuals and family or friend carers to identify people's strengths, networks, and community assets. You will remain up to date and compliant with all relevant legislation and adhere to organisational procedures, policies and codes of conduct as well as Social Work England professional standards.
