Job description
The Care Home Practitioner will work as part of the Integrated Community Team, be multi-skilled and will work across the various disciplines. The Care Home Practitioner will provide evidence based up to date training and education within the care homes across MSE.
The Care Home Practitioner will undertake a wide range of interventions and supporting actions such as working with hydration and nutrition tools and care home assistant education. They will support care home to deliver quality care and refer where appropriate.
To work as an autonomous practitioner within the care home team, supporting the delivery of the EPUT vision of being the leading health and wellbeing service in the provision of mental health and community Service and Liaises with other agencies on the Education, training and competencies of care home staff.

Clinical/Professional Duties: The role of the Care Home Practitioner will include:
• To undertake clinical training and practice development within identified care homes within remit.
• To work in collaboration with Care home managers to deliver various training to care home staff including the use of Whzan.
• To provide professional support and advice to staff in the Care home setting, and to facilitate education and training to registered care homes using evidenced based care.
• To plan, implement and evaluate Education needs in care homes involving District Nurses, EPUT care home team members and care home staff
• To develop education packages using research and current clinical evidence with particular emphasis to the care home needs for training to avoid unnecessary hospital admissions.
• To actively contribute to the delivery of the NHS and Midlands ambition, to eliminate all avoidable pressure ulcers and reduce 999 calls and ambulance conveyances within the care homes
• Work in partnership with all members of the integrated team both internally and externally to the Trust to ensure continuity of care and training and to avoid duplications.
• Attend MDT meetings and care home meetings when required.
• Support the assessment and management of residence care needs through monitoring of progress, feedback discussions, reports and multi-disciplinary meetings.
• Work with and alongside care home staff, relatives and carers, sharing knowledge and skills to promote understanding of the aims of intermediate care and/or rehabilitation to ensure a consistent approach to care provision.
• Perform extended skills following training and assessment of competency with registered practitioner.
• Establish a therapeutic relationship with the patient, carers and relatives and care home staff to engage them in health promotion, prevention and rehabilitation processes.
• Adapt communication style to meet the needs of the individual to breakdown any existing barriers to communication. Using appropriate methods to convey messages and support the expression of thoughts and ideas.
• Develop effective working relationships and work collaboratively with medical, nursing, therapy and care homes colleagues to provide an integrated and coordinated service.
• Under the direction of the relevant professional appropriately and effectively communicate to patients, carers and relatives, information that may be sensitive.
• Use strategies to adapt to barriers such as cultural beliefs and language deficit.
