- Conduct comprehensive reviews of care packages to ensure optimal support for individuals.
- Facilitate smooth and timely hospital discharges, minimizing delays and ensuring appropriate post-discharge arrangements.
- Maintain registration with Social Work England to uphold professional standards and ethics.

Requirements:

- Qualified and registered with Social Work England.
- Strong communication and interpersonal skills.
- Ability to work effectively within multidisciplinary teams.
- Knowledge of relevant legislation and policies in social care and healthcare settings.
