Job Overview:

The successful candidate will join a dynamic healthcare team, contributing to the delivery of exceptional rehabilitation services within the community setting.
Key Responsibilities:
Assessment and Treatment:
- Conduct comprehensive assessments of patients’ physical and functional abilities.
- Develop and implement individualized treatment plans based on assessment findings.
- Provide evidence-based physiotherapy interventions to promote optimal recovery.
Community Rehabilitation:
- Deliver physiotherapy services in various community settings, including patients’ homes.
- Collaborate with multi-disciplinary teams to ensure holistic and patient-centered care.
- Facilitate patient independence and community reintegration through effective rehabilitation strategies.
Documentation and Reporting:
- Maintain accurate and up-to-date patient records in accordance with professional and organizational standards.
- Prepare clear and concise progress reports, contributing to effective communication within the healthcare team.
Education and Support:
- Educate patients and their families on therapeutic exercises and self-management strategies.
- Provide guidance and support to promote patient understanding and compliance with treatment plans.
Professional Development:
- Stay informed about current trends and advancements in physiotherapy practice.
- Participate in relevant training and development opportunities to enhance skills and knowledge.
Qualifications and Experience:
- Bachelor’s degree (or equivalent) in Physiotherapy.
- Registration with the Health and Care Professions Council (HCPC).
- Previous experience as a Band 6 Physiotherapist, preferably in a community rehabilitation setting.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a collaborative team.
- Commitment to continuous professional development.
