Job description
This post is a new role within the departments Mental Health Teams. The primary role will be to assess new referrals into the Teams and to ascertain if the needs of an adult would be best met in one of our Mental Health Teams or by a Locality Team.
This will involve assessing the individual, liaising with carers and families, speaking to other professionals e.g GP etc and, following discussions with the manager and other Team members, deciding which Team within the department would be most appropriate to meet the needs of the adult.
You will support adults to have a good assessment, clearly identifying needs whilst considering the context that the adult lives in and listening to their voice. You will then plan to help them meet their needs by looking at informal and community based support in addition to providing statutory services to meet their needs.
You will also work with adults to safeguard them against abuse and neglect. In addition to the above the post holder would hold a small caseload and partake in duty work.
To help us recruit social workers who are committed to working in Middlesbrough we are currently offering a 10% supplement to social work salaries. This additional 10% will be reviewed at the end of March 2024.
The above post is subject to an enhanced Disclosure and Barring Service (DBS) check.
