Job description
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Plan, implement and evaluate individualised specialist patient care programmes based on evidence based practice and the application of clinical reasoning skills Promote and support self-management and engage with patient support groups To be professionally and legally accountable for all aspects of own work including management of patients in your care. To organise the caseload efficiently and effectively with regard to clinical priorities and use of time Promote better understanding of Diabetes for patients, their families and carers and other agencies involved with the patient through education, support and counselling. To delegate care of patients, as appropriate, to assistant staff and supervise their work Recognise and act on signs, symptoms and complications of diabetes conditions To manage a caseload of patients with complex Diabetes needs, using evidence based practice and patient centred goals to assess plan implement and evaluate inventions To deliver accredited Type 2 (DESMOND) Structured Education to newly diagnosed patients To attend regular team meetings and other meetings as required. *DVLA have a number of reciprocal arrangements with overseas countries, for further information please visit the DVLA website
