Job description
To be flexible in working hours in order to support initiatives which aim to reach immunisation key performance indicators Actively promote the health of individuals and the community by participating in child and adult health promotion programmes and public health initiatives to improve immunisation uptake. Ensure efficient and effective use of resources to minimise incurred expenditure eg mileage, telephone calls, use if stationary etc To be aware of and report Safeguarding issues to safeguard and protect the health and wellbeing of children, young people and vulnerable adults working within safeguarding children policies and guidelines Contribute to clinical audit, research and clinical governance To assist in the monitoring, development and review of quality measures and key performance indicators. To travel to and from educational establishments, community venues and children and young peoples homes, transporting essential equipment required for delivery of immunisations Contribute to clinical audit, research and clinical governance as part of the Immunisation Team. Observe the complaints and disciplinary procedures in accordance with the Trust policy. Report sickness absence and return relevant forms to line manager. Clinical Maintain active status on NMC register acting in accordance with the professional code and guiding documents in adherence with Revalidation Framework. Be aware of current working practices in accordance with Trust policies and procedures Lead in the organisation of the planning, delivery and evaluation of immunisation and vaccination programmes to school age children including ordering and delivery of supplies and equipment, storage of vaccines and waste disposal, delegating duties as appropriat Ensure correct ordering, storage and handling of vaccines procedures are followe Identify and plan uptake immunisations for those with incomplete immunisations opportunistically and on referral from the wider children and young people workforce Screen clients to ensure their inclusion for immunisation meets the required criteria in the Patient Group Direction (PGD) Obtain informed consent and act on clinical judgements formed from complex information Prepare and administer the vaccine safely and competently in accordance with local/national standards, guidelines and policies. Respond to clinical emergencies (anaphylaxis ) initiating basic life support ensuring emergency procedures are carried out and reported in accordance with local/national standards, guidelines and policies. To be named / link nurse for immunisations within specific educational establishments Supervise children and young people during immunisation sessions Report any adverse reactions in accordance with the yellow card reporting system Undertake opportunistic and domiciliary immunisations, following up non immunised children; providing information, advice and updating data as required. Ensure any vaccination given is recorded as per local guidelines/policy Assess and signpost children and young people to the appropriate named SCPHN where additional advice and support is indicated for identified health needs. Liaises and advises teachers and parents of the health needs of children and young people following vaccination. To adapt practice to meet individual circumstances with due regard for cultural, social and linguistic differences Leadership: Develop and contribute to the development and implementation of policies, procedures and guidelines relevant to own area of work. Provide advice where services are developing. Manage their time effectively to ensure prioritisation of workload in order to achieve agreed deadlines and service is covered. Initiate incident reporting and take appropriate action following Bridgewater policies Submit statistical returns and compile reports as necessary Use their own professional judgement to identify solutions to a range of day-to-day problems, seeking advice where appropriate from the SCPHN Supervision and Development Participate in and complete the agreed induction programme for newly appointed staff Provide mentorship, preceptorship and clinical supervision to staff as appropriate Act to maintain an appropriate learning environment to meet the educational and developmental needs of student nurses and other learners as required Support the development of the team by teaching and assessing junior members of the team Undertake mandatory and statutory training that support the post holders role Undertake training specific to the post Participate in individual performance reviews (IPA) and clinical supervision. Be responsible for own professional development, maintaining a portfolio of evidence to demonstrate competency and continuing professional development, making sure that steps are taken to maintain adequate skills and knowledge base Support the delivery of practical competency assessment of nurses undertaking vaccination and immunisation training. Communication Maintain contemporaneous and accurate records maintaining confidentiality, in accordance with NMC and Trust policies and guidelines Use Trust documentation to maintain accurate records of visits, mileageMaintain a Bridgewater Community Healthcare NHS Trust diary (paper or electronic calendar) for the purpose of planning work. Diaries may be requested to be submitted for audit purposes Develop a working knowledge and experience of IT systems, including SystmOne Adhere to the clear desk/screen policy Communicate effectively with individuals, families and professionals in planning and delivering care. Utilise a range of communication methods with colleagues and managers to include electronic communication. Maintain effective working relationships, work effectively and support team members. Participate in formal and informal ways of communication both within the team and the wider primary care setting where appropriate. Work in partnership with school staff, health colleagues and other agencies e.g. Education and Social Services. Attend staff meetings and ensure that minutes are read and signed, especially when attendance has not been possible
