Remuneration: Rate @ 50%
Description: Full or Part time Private Dentist required for 2-5 days a week in a busy, predominately private practice near Launceston, Cornwall.
• Mon to Fri - 35 hours per week
• 50% split on private and all labs
• Interest in Endodontics desirable but not essential
• Modern, fully computerised surgeries
• Digital x-rays
• OPG
• Rotary Endo
• Full support team with Hygienist
• Available from now, start date negotiable

Compliance requirements:
- Full NHS Performer Number
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
