Job description


We are seeking a dedicated Band 5 Community nurse to join our dynamic team. An exciting and new opportunity has arisen for a forward thinking and self-motivated individual to join our Community Team.

Join us in making a positive impact on the health and wellbeing of our community and contribute to the delivery of outstanding nursing care.

In your role you will focus on providing high-quality nursing care in our local community and residential care homes across Heywood, Middleton, Rochdale. Prevent hospital admission, support hospital discharge home, virtual ward monitoring and promote the health and wellbeing within the community.

Support the team in developing a new service to provide improved health outcomes through early detection and intervention. Enhance accessibility to healthcare services, reducing hospital conveyances. Empower residents and care staff to actively manage health, promote ageing well, well-being, and contribute to a healthier ageing population.

Provide and develop bespoke education and training to care homes staff and provide support for care home staff in providing the highest quality care.

The post holder will undertake professional duties for residents residing within all residential care homes within the borough providing comprehensive health care sensitive to the needs of the ageing population.

To undertake planned and opportunistic health promotion and education activities with residents and carers

Conduct comprehensive nursing assessments for residents, identifying their healthcare needs and developing care plans.

Deliver evidence-based nursing interventions and treatments to meet the physical and emotional needs of residents.

Collaborate with multidisciplinary teams, including doctors, therapists, District Nurses and social workers, to ensure holistic care for residents.

Provide education and support to residents and their families on health management, preventive care, and lifestyle choices.

Support Care Home Managers and staff with creating care plans, monitor and evaluate the effectiveness, adjusting interventions as necessary to achieve optimal outcomes.

Participate in health promotion activities and community outreach to enhance public awareness and engagement.

Foster a positive and collaborative working environment within the residential care homes, promoting a person-centred approach.

Uphold the NHS and NMC values and maintain a commitment to delivering compassionate and respectful care to all residents.

Act as a patients advocate and consider safeguarding where appropriate.

To take responsibility for the day-to-day management of your nursing workload and be proactive in managing own workload.

To establish priorities of care in accordance with caseload management.

To take responsibility for updating and developing professional knowledge and skills which relate to clinical practice.

To act within the NMC Code of Professional Conduct and comply with the Trust’s policies and procedures.

To participate in the development, training and mentorship of preregistration students and staff as required including induction and preceptorship programmes.

To provide skilled clinical nursing practices, collaborate with care home staff to ensure care is tailored to the individual needs of the resident.

Maintain effective communication with residents, families and colleagues to ensure a holistic and person-centred approach.

Engage in ongoing professional development activities to stay informed about current nursing practices, regulations and emerging trends within community and residential care homes.

Educate residents and care staff on health promotion, preventative care, self-management strategies to enhance overall well-being. Support and facilitate health education programs within the community to promote a healthy lifestyle.

This vacancy is open to those who have a current NMC PIN to practice as an Registered Nurse or Midwife in the UK, or those who are currently enrolled on a course which means they expect to receive their full NMC PIN within the next 3 months. Please note an NMC PRN PIN is not a full NMC PIN and would not fulfil the essential criteria for this role.

The NCA can no longer accept applications from International Nurses and Midwives who are yet to complete all competencies needed to receive a full NMC PIN to practice in the UK as we have paused our OSCE programme indefinitely.
