Job Title: B8a Advancing Practice Dietician in Paediatric Diabetes
Reporting to: Therapy Services
Grade: Band 8a
WTE: 22.5 hours per week
Are you interested in working as a highly specialist APP for Children and Young People at Barnet and Chase Farm Hospitals?
This is an exciting new opportunity to contribute to the development of the Paediatric APP role, participate in level 7 study and provide high level clinical care and support to a Diabetes Children and Young People’s caseload.
The successful candidate will work as an autonomous practitioner providing highly specialist assessment and management of newly diagnosed type 1 diabetic children and young people and continued management through their journey. They will have excellent knowledge of current evidence based practice and used advanced clinical reasoning skills.
In addition they will be providing support to the B6 and B7 Dieticians providing clinical supervision and training. The role will also provide support to the Children and Young People’s Clinical lead.
Participating in an ACP or Msc pathway will be an essential part of the role.

• To develop the Children and Young People’s (CYP) Diabetes Dietetic Clinical Lead role to Advanced Clinical Practitioner (ACP)
• To work as an autonomous practitioner providing advanced CYP Diabetes Dietetic assessment and management to a specific, highly specialised clinical caseload
• To work as an advanced practitioner, using assessment, requesting of investigations and advanced clinical reasoning skills, to assist in the formulation of a diagnosis.
• To develop comprehensive management and treatment plans, utilising a wide range of treatment skills to formulate a specialised empowering and progressive programme of care.
• To work with the MDT as a vocal member of the team contributing essential information to the service. To actively communicate with internal and external referrers, Paediatric consultants, CAMHS, Education and relatives and carers on the most appropriate course of intervention.
• To provide and facilitate high quality education and training to health professionals both within and external to the trust.
• To assist the CYPT Clinical Lead in supporting the team in the day-to-day operational management of the designated physiotherapy service.
• To plan, implement and monitor strategic delivery of a highly specialist, evidence based clinically effective service in conjunction with the CYPT Clinical Lead.
• To participate in an MSc/ACP pathway appropriate to specialist role
We are a friendly supportive team of Children and Young People Physiotherapist and Dieticians and are looking for an experienced, knowledgeable, innovative team player with excellent clinical and communication skills to join our team.
• To be professionally and legally responsible and accountable for all aspects of own professional activities in line with the standards of the Trust, Department and the British Dietetic Association (BDA) and Health Care Professionals Council (HCPC)
• To have overall accountability for the planning and management of a highly complex and specialist caseload of patients and to organise this effectively and efficiently with regard to clinical priorities and use of time.
• To utilise advanced clinical reasoning skills and evidence based knowledge to assess patients with highly complex presentations using a bio-psychosocial model and then to implement dietetic management plans as an autonomous practitioner.
• To be able to explore complex and distressing personal histories, in a sensitive and supportive manner
• To continuously assess, evaluate and modify treatment aims, objectives and desired outcomes in collaboration with the patient, any carers, and the multidisciplinary team.
• To use highly advanced communication and behaviour change skills to facilitate a lifelong self management approach with patients living with a chronic disease
• To use cognitive behavioural principles to increase a patient’s dietary knowledge incorporating, as appropriate but not exclusive to, education and goal setting,
• To attend and actively participate in multidisciplinary team meetings, discussing patients’ clinical management
• To make recommendations to colleagues and members of the MDT based on advanced theoretical and practical knowledge.
• To be aware of issues affecting patients with chronic disease with particular reference to specialist services, support mechanisms that are available within the community for groups from diverse ethnic and cultural backgrounds.
• To suggest the need for appropriate specialist investigations
• To be a source of expert clinical opinion and support to the other peers in the dietetic team
• To produce up to date, clear evidence-based resources for patients.
• To ensure that patients have consented to dietetic interventions in line with the Trust and department ethico-legal policies and the British Dietetic Association Professional Code of Conduct.
• To be responsible for ensuring written and electronic patient records are updated as required and statistical activity data records are produced in accordance with professional, departmental and Trust standards
• Professionally and legally responsible for provision of highly specialist dietetic assessment and intervention to individual patients with highly complex needs.
