This is a temporary role focused on assessing and reviewing adult residents to ensure safe discharges from both hospital and community settings.

Key responsibilities include:
Conducting assessments and reviews of adult residents in Bedford Borough.
Ensuring safe discharges from hospital and community settings.
Utilising electronic record-keeping systems effectively.

Qualifications and Experience:

Experience in electronic record-keeping is essential.
