Key Responsibilities:
- Conduct assessments and develop tailored treatment plans for clients in the community setting.
- Work closely with other healthcare professionals to provide holistic care.
- Implement evidence-based interventions to address physical and mental health needs.
- Collaborate with patients to promote self-management and well-being.
- Participate in multidisciplinary team meetings and contribute to care planning.
Qualifications:
- HCPC registered Physiotherapist with Band 6 experience.
- Proven experience in community-based settings.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Commitment to ongoing professional development.
