Are you looking for a full time Monday-Friday role ?

Active Care Group is the leading provider of complex care. Within the Care in the Home Division, we offer person-centered care in the home, either through live in or live out care packages to people with complex health needs, spinal cord injury and acquired brain injury.

We are looking for a Care Manager to be responsible for care packages in the Bedfordshire / Buckinghamshire / Hertfordshire / Oxfordshire areas.

As a Care Manager, you will have ownership of your clients care packages. This will include assessing and reviewing clients needs, liaising with relevant external bodies and internal teams, and training and developing the Support Workers.

Location: Home based with travel in designated region (Bedfordshire/Buckinghamshire/Hertfordshire/Oxfordshire). You must be a driver with a car.

Hours: 37.5 hours per week, predominantly Monday-Friday 9am-5.30pm, however these hours and days may need to worked flexibly.You must also be willing to be part of an on-call rota on nights/weekends.
