You will be able to find a full Job description and Person Specification attached within the supporting documents or please click Apply now to view in Trac To assess the social, developmental, educational and health needs of individual young people within the Nephrology and Transplant service and deliver appropriate programmes and activities. To establish professional relationships with key individuals and as appropriate parent/carers and siblings. To participate in the multidisciplinary team using specific skills to contribute to the care and understanding of young people, their families and carers. To establish strong relationships with appropriate professionals within the Adult and Paediatric clinical specialist teams and partnership organisations relevant to the role. To create opportunities for young people to participate in service development and actively share their views within the service. To support the management of the transition of young renal patients between paediatric and adult services and work in partnership with the specialist teams. To participate in multidisciplinary team meetings, reviews etc providing professional advice on adolescent issues, reporting any relevant information or concerns as necessary, making referrals and maintaining appropriate links.
