Job description

A Vacancy at St George's University Hospitals NHS Foundation Trust.

• To engage with patients with diabetes at the earliest opportunity when they are listed for elective and emergency surgery, with a focus on preparation for surgery and prevention of adverse events
• To work as an autonomous, advanced practitioner and effectively deliver an expert nursing service within the preoperative assessment area
• To effectively manage a caseload of patients/clients
• To actively develop evidence based clinical nursing practice in line with current research and guidelines
• To work within the specialty Care Group to coordinate and manage a comprehensive, patient centred, nurse led pre assessment service.

• To carry out detailed preoperative assessment of elective patients, including initiation of discharge planning procedures.
• To carry out/ initiate preoperative investigations/diagnostic tests in line with existing pre-assessment protocols.
• To be responsible for following up and reviewing investigation results and alerting appropriate clinicians as necessary.
• Using relevant algorithms and risk assessment tools to identify areas of clinical concern, referring on to relevant specialists as per set criteria.
• To be responsible for ensuring excellent documentation of patient assessment, planned care and patient information provided at pre assessment.
• To liaise closely with pathway coordinator, Ward Manager, CNS, Discharge Coordinator, Bed Manager with regard to complex admission and identified discharge problems.
• To support patients and their families, involving them in planning of admission and care where appropriate.
• To work in collaboration with other health care professionals to develop and implement new and innovative tools to improve perioperative care for patients with diabetes including screen and supporting diabetes treatment plan
• To work with patients and their carers to develop individualised care plans providing knowledge and support enabling them to self-manage their diabetes during the perioperative pathway
• To provide support for all grades of nursing staff in relation to diabetes nursing, with a specific focus on perioperative diabetes care and ensuring a safe framework for practice is in place and maintained
