We are currently seeking a skilled Band 6 CAMHS Practitioner Transition Worker for a temporary assignment in the Child and Adolescent Mental Health Services (CAMHS) sector.
Position Overview: As a Band 6 CAMHS Practitioner Transition Worker, you will play a crucial role in facilitating the transition of young individuals within the CAMHS system. This is a temporary position for a duration of 3 months, providing an exciting opportunity to make a positive impact on the lives of young people during a critical phase of their mental health journey.
Key Responsibilities:
Transition Support: Collaborate with the CAMHS team to facilitate a smooth transition for young individuals between different stages of the mental health care system.
Assessment and Intervention: Conduct comprehensive assessments to identify the unique needs of each individual and provide timely interventions to support their mental health transition.
Collaboration with Multi-disciplinary Team: Work closely with a multi-disciplinary team, including psychiatrists, psychologists, social workers, and other healthcare professionals, to ensure coordinated and holistic care.
Case Management: Manage a caseload of individuals, ensuring that appropriate care plans are developed, implemented, and reviewed regularly.
Education and Support: Provide education and support to individuals and their families, empowering them to actively participate in the transition process.
Documentation: Maintain accurate and up-to-date records of assessments, interventions, and progress notes in accordance with professional and organizational standards.
