We are currently seeking a skilled Band 6 CAMHS Practitioner Transition Worker for a temporary assignment in the Child and Adolescent Mental Health Services (CAMHS) sector.

Position Overview: As a Band 6 CAMHS Practitioner Transition Worker, you will play a crucial role in facilitating the transition of young individuals within the CAMHS system. This is a temporary position for a duration of 3 months, providing an exciting opportunity to make a positive impact on the lives of young people during a critical phase of their mental health journey.

Key Responsibilities:

  1. Transition Support: Collaborate with the CAMHS team to facilitate a smooth transition for young individuals between different stages of the mental health care system.

  2. Assessment and Intervention: Conduct comprehensive assessments to identify the unique needs of each individual and provide timely interventions to support their mental health transition.

  3. Collaboration with Multi-disciplinary Team: Work closely with a multi-disciplinary team, including psychiatrists, psychologists, social workers, and other healthcare professionals, to ensure coordinated and holistic care.

  4. Case Management: Manage a caseload of individuals, ensuring that appropriate care plans are developed, implemented, and reviewed regularly.

  5. Education and Support: Provide education and support to individuals and their families, empowering them to actively participate in the transition process.

  6. Documentation: Maintain accurate and up-to-date records of assessments, interventions, and progress notes in accordance with professional and organizational standards.

Qualifications and Experience:

  1. Qualified and registered Mental Health Practitioner (Band 6 or equivalent).
  2. Significant experience working within Child and Adolescent Mental Health Services (CAMHS).
  3. Knowledge of mental health transition processes and challenges faced by young individuals.
  4. Excellent communication and interpersonal skills.
  5. Ability to work collaboratively within a multi-disciplinary team
